Middlesex Professional Services Foundation







The Middlesex Professional Services Foundation (MPSF) aims to facilitate secure communication and sharing of health information among physicians, hospitals, laboratories, pharmacies and other providers to improve the health care for all Middlesex County residents.  The Foundation was awarded a $1 Million grant in 2006 from the Physicians' Foundation for Health Systems Excellence to support the Clinical Integration Project.

 Clinical Integration Project


The MPS Foundation's Clinical Integration Project aims to improve healthcares services delivered to residents of Middlesex who have certain chronic illnesses through the development of an electronically and clinically integrated network of independent physicians.  The MPS Foundation will facilitate communication and sharing of health information among physicians, hospitals, laboratories, pharmacies and other providers through the use of web-based Continuity of Care and Referral System. (CCRS).


The major goals of the project are: 

  • Continuity of Care Record (CCR).  Our goal is to create a CCR for every resident of Middlesex County.  The CCR is a new national standard that has up to 200 data fields including patient demographics, medications, allergies, and lab results. 
  • Electronically linking every healthcare physician in Middlesex County regardless of business affiliation so the CCR can “follow the patient”, wherever they go. 
  • On-line Physician Referral system should greatly improve the efficiency of Physician-to-Physician communications and hospital discharge. 
  • On-line lab results for chronic diseases and managed conditions Quest with full lab viewer available through direct web links. 
  • A multi-condition Chronic Care Management System and Patient Registry from DocSite, an award-winning application, to track chronic diseases and other conditions relevant to your practice.
